At the Dykema DSO Conference 2024 this July, DSO leaders shared their top six areas of focus for the next six months.<\/em><\/p>\n<p class=\"pb-2\">This year has been like a tsunami, with a deluge of difficult business conditions including high supply costs, high labor costs, high interest rates, and low insurance reimbursements overwhelming many dental practices.<\/p>\n<p class=\"pb-2\">At the <a href=\"https:\/\/www.dykema.com\/\" target=\"_blank\" rel=\"noopener\">Dykema<\/a> DSO Conference in Denver on July 10, dental group executives shared how they\u2019re faring and the top six areas where they&#8217;re focusing their attention for the rest of the year.<\/p>\n<h2 class=\"pb-2\"><strong>1. Standardization, Interoperability, and Visibility of Data<\/strong><\/h2>\n<p class=\"pb-2\">One hot topic at the Dykema DSO conference was standardization and efficiency.<\/p>\n<p class=\"pb-2\">\u201cGetting all of our practices on one unified, <a href=\"https:\/\/www.planetdds.com\/blog\/why-your-dental-practice-management-solution-is-better-in-the-cloud\/\" target=\"_blank\" rel=\"noopener\">cloud-based system<\/a> was absolutely imperative for us. We\u2019re completing our rollout of the <a href=\"https:\/\/www.planetdds.com\/solutions\/denticon\/\" target=\"_blank\" rel=\"noopener\">Denticon<\/a> Practice Management system across all of our GP practices and adding <a href=\"https:\/\/cloud9.software\/\" target=\"_blank\" rel=\"noopener\">Cloud 9<\/a> practice management for ortho right after that,\u201d said Jason Heffelfinger, CEO of <a href=\"https:\/\/www.dentalcarealliance.net\/\" target=\"_blank\" rel=\"noopener\">Dental Care Alliance<\/a>, a DSO with more than 400 affiliated practices in 26 states. With both Denticon and Cloud 9 produced by Planet DDS, the platforms are integrated so patient files can be entered or accessed from either application.<\/p>\n<p class=\"pb-2\">\u201cWhen you have one system that you can utilize across all your practices, it just makes it easier for everybody to execute. When we think about staffing, doctor days, data\u2014it just makes it so much easier to understand where you have opportunities. Then you can thank them for the work they do, and of course, you can course-correct when necessary,\u201d Heffelfinger explained.<\/p>\n<p class=\"pb-2\">AJ Acierno, DDS, president of clinical operations for <a href=\"https:\/\/smilebrands.com\/\" target=\"_blank\" rel=\"noopener\">Smile Brands<\/a>, agreed. The company has more than 650 affiliated practices in 32 states.<\/p>\n<p class=\"pb-2\">\u201cWe\u2019re an acquisition-based company and that means we inherited a ton of different systems,\u201d Dr Acierno said. \u201cIt means everything to be able to standardize the technology and have all your data all in one place. It lets you measure metrics, apples to apples.\u201d<\/p>\n<h2 class=\"pb-2\"><strong>2. Increasing Same-Store Growth Through Systems<\/strong><\/h2>\n<p class=\"pb-2\">The company <a href=\"https:\/\/www.oms360.com\/\" target=\"_blank\" rel=\"noopener\">OMS360<\/a> drove 40% organic growth in the first half of 2024 across its oral surgery business after implementing a new patient workflow, said its CEO, Trevor Maurer.<\/p>\n<p class=\"pb-2\">\u201cWe wanted to understand why some of our surgeons were more efficient than others, so we performed a comprehensive analysis of surgeons and workflows. We literally did a time study of our <a href=\"https:\/\/www.planetdds.com\/blog\/9-tips-for-improving-the-dental-patient-journey\/\" target=\"_blank\" rel=\"noopener\">patient journey<\/a>, the staff, and even the surgeons. We needed to know who was doing what, and the impact that had on the entire patient experience. From this, we identified six phases of the patient journey and detailed how long each took for our highest-performing practices,\u201d Maurer said.<\/p>\n<p class=\"pb-2\">When the research came back, the data showed that it wasn\u2019t just something the surgeons were doing but the systems they had in place that made them successful. OMS360 then modeled the stages across its practices:<\/p>\n<ul class=\"my-2\">\n<li class=\"list-disc ml-8\">Patient access<\/li>\n<li class=\"list-disc ml-8\"><a href=\"https:\/\/www.planetdds.com\/blog\/scheduling-for-dso-growth-across-multiple-dental-offices\/\" target=\"_blank\" rel=\"noopener\">Scheduling<\/a><\/li>\n<li class=\"list-disc ml-8\">Pre-appointment, such as insurance verification<\/li>\n<li class=\"list-disc ml-8\">Consultation and treatment planning<\/li>\n<li class=\"list-disc ml-8\">Treatment<\/li>\n<li class=\"list-disc ml-8\">Account reconciliation<\/li>\n<\/ul>\n<p class=\"pb-2\">\u201cWe systemized the six phases and let each team know where the inconsistencies are in their methods. Not everyone will want to make a change, and it is truly their choice,\u201d Maurer said. \u201cOur commitment is to enable the doctors to practice at the top of their license.\u202fIf they want to do it, OMS360 can get them there.\u202fSurgeons that were under $4 million have increased to over $6 million annually.\u201d<\/p>\n<p class=\"pb-2\">\u201cWe\u2019ve been focusing on same-day care initiatives,\u201d said Teresa Williams, the chief financial and operations officer of <a href=\"https:\/\/www.thedentalexpress.com\/\" target=\"_blank\" rel=\"noopener\">Dental Express<\/a>, a dental group with six affiliated practices in California. \u201cWe\u2019re taking a hard look at what\u2019s happening for each of our patients when they\u2019re in the building. People want to get care immediately, so we\u2019re making it easy for them through third-party financing, building larger teams so we can provide same-day care, and even expanding the footprint of our offices.\u201d<\/p>\n<h2 class=\"pb-2\"><strong>3. Same-Store Growth Through Payer Mix Changes<\/strong><\/h2>\n<p class=\"pb-2\">Many DSO leaders predicted the COVID-19 impact on the global and local economy, from supply chain issues to higher prices for goods, higher wages, and higher interest rates. Himesh Kana, DMD, managing director of <a href=\"https:\/\/dentaldepotdfw.com\/\" target=\"_blank\" rel=\"noopener\">Dental Depot DFW<\/a>, began evaluating their systems and payor mix in 2022. Dental Depot has 37 affiliated practices in Texas, Oklahoma, Arizona, and Missouri.<\/p>\n<p class=\"pb-2\">\u201cWe looked at our phones, we looked at our treatment acceptance rate, we looked at training, we looked at all of our payors, we looked at our data, and we looked at our patient volume,\u201d Kana said. \u201cWe have a really solid membership model with <a href=\"https:\/\/subscribili.com\/\" target=\"_blank\" rel=\"noopener\">Subscribili<\/a>; it\u2019s now our second highest segment of commercial payors, and our per visit output on our membership model is much higher, by 20% to 25%.\u201d<\/p>\n<p class=\"pb-2\"><a href=\"https:\/\/thesmilist.com\/\" target=\"_blank\" rel=\"noopener\">The Smilist Management<\/a>, a DSO with almost 100 affiliated practices in the Northeast, is focusing on revenue cycle management to drive same-store growth too.<\/p>\n<p class=\"pb-2\">\u201cWe negotiated a rate increase on our PPO plans. We tightened up on our <a href=\"https:\/\/www.planetdds.com\/blog\/creative-ways-dsos-are-solving-the-revenue-cycle-management-rcm-challenge\/\" target=\"_blank\" rel=\"noopener\">RCM processes<\/a> and our accounts receivable,\u201d said Thomas Passalacqua, director of business development. \u201cWe added automation to our RCM, which has really improved patient communication, insurance verification, and collection rates.\u201d<\/p>\n<p><script src=\"https:\/\/fast.wistia.com\/embed\/medias\/trewmk34zk.jsonp\" async><\/script><script src=\"https:\/\/fast.wistia.com\/assets\/external\/E-v1.js\" async><\/script><\/p>\n<div class=\"wistia_responsive_padding\" style=\"padding: 56.25% 0 0 0; position: relative;\">\n<div class=\"wistia_responsive_wrapper\" style=\"height: 100%; left: 0; position: absolute; top: 0; width: 100%;\">\n<div class=\"wistia_embed wistia_async_trewmk34zk seo=true videoFoam=true\" style=\"height: 100%; position: relative; width: 100%;\">\n<div class=\"wistia_swatch\" style=\"height: 100%; left: 0; opacity: 0; overflow: hidden; position: absolute; top: 0; transition: opacity 200ms; width: 100%;\"><img decoding=\"async\" style=\"filter: blur(5px); height: 100%; object-fit: contain; width: 100%;\" src=\"https:\/\/fast.wistia.com\/embed\/medias\/trewmk34zk\/swatch\" alt=\"\" aria-hidden=\"true\" \/><\/div>\n<\/div>\n<\/div>\n<\/div>\n<h2 class=\"pb-2\"><strong>4. Artificial Intelligence to Increase Case Acceptance<\/strong><\/h2>\n<p class=\"pb-2\">Technology is another area that dental support organizations are leveraging to increase same-store growth.<\/p>\n<p class=\"pb-2\">\u201cOne of the newest technologies we\u2019ve implemented is <a href=\"https:\/\/hellopearl.com\/\" target=\"_blank\" rel=\"noopener\">Pearl AI<\/a>, which integrates directly into our Apteryx digital imaging,\u201d says Bradley Dykstra, founder of MI Smiles Dental Group, which has seven affiliated locations in Michigan. \u201cA picture\u2019s worth a thousand words, and it\u2019s really helped the patients to visualize what we\u2019re talking about. The clinicians like it, and it\u2019s a time-saver for the hygienists because it shows where there is radiographic bone loss. For me, it\u2019s a great way to gauge how each of the associates are diagnosing.\u201d<\/p>\n<p class=\"pb-2\">\u201cWe have recently partnered with <a href=\"https:\/\/www.overjet.com\/\" target=\"_blank\" rel=\"noopener\">Overjet<\/a> to incorporate AI into our operational and clinical processes,\u201d said Kyle Smith, chief financial officer at My Community Dental Centers, a non-profit dental support organization with almost 50 locations in Michigan. The outcomes have been impressive, showcasing enhanced diagnostic accuracy, streamlined claim submissions, and improved reimbursements from payers.\u201d<\/p>\n<p class=\"pb-2\">DSO leaders are evaluating other AI-powered technologies including call center bots.<\/p>\n<p class=\"pb-2\">\u201cThis new generation (of patients) doesn\u2019t want to deal with people, right?\u201d explained Dr. Kana. \u201cIt\u2019s inevitable that in six to seven years, we\u2019re going to see a lot of bots handling phone calls and bot-trained scheduling.\u201d<\/p>\n<h2 class=\"pb-2\"><strong>5. New Dental Technologies Elevating Patient Care<\/strong><\/h2>\n<p class=\"pb-2\">A lot of the discussions at Dykema centered around new technologies and the results people have seen.<\/p>\n<p class=\"pb-2\">Dental Care Alliance is using Guided Biofilm Therapy (GBT), which discloses the location of plaque and removes it using air and piezo technology from EMS Dental instead of traditional scaling, root planning, and polishing paste.<\/p>\n<p class=\"pb-2\">Dental Express has expanded its intraoral scanner technologies and is looking to bring in AI-based scheduling technology to reduce no-shows and last-minute cancellations.<\/p>\n<p class=\"pb-2\">Jill Nesbitt, the founder of <a href=\"https:\/\/www.optimizedent.com\/\" target=\"_blank\" rel=\"noopener\">Optimize Dental Consulting<\/a>, hosted a technology workshop at the Dykema DSO Conference with more than forty DSO leaders from operations and IT. They discussed the emergence of new dental analytics companies that pull in data from multiple software systems including accounting, human resources, call centers, and practice management systems to help dentists and managers have a holistic view of the data driving their profit and loss statements.<\/p>\n<p class=\"pb-2\">Some practice management systems like Denticon are moving away from branding themselves as an all-in-one solution and to a dental operating system model, where dental practices can choose to read, share, and write back information with third parties via secure <a href=\"https:\/\/www.planetdds.com\/blog\/planet-dds-api-program\/\" target=\"_blank\" rel=\"noopener\">APIs<\/a>.<\/p>\n<p class=\"pb-2\">\u201cPeople want the freedom to be able to integrate different software solutions into their practice management system. You see that with RCM, for example, said <a href=\"https:\/\/fischerspro.com\/p\/dental-consultant-Orlando-Fl-Dee-Fischer-p47450.asp\" target=\"_blank\" rel=\"noopener\">Dee Fischer<\/a>, the co-founder of a DSO with six affiliated locations and one of the best-known consultants for group practices. &#8220;They want to be able to verify insurance, submit claims, and reconcile payments all within their PMS, without having to launch another application.\u201d<\/p>\n<h2 class=\"pb-2\"><strong>6. Staffing Shortages and the Importance of Culture<\/strong><\/h2>\n<p class=\"pb-2\"><a href=\"https:\/\/www.planetdds.com\/blog\/overcoming-dental-staffing-shortages\/\" target=\"_blank\" rel=\"noopener\">Staffing shortages<\/a> remains an issue for almost every dental group, especially when it comes to recruiting hygienists and dental assistants.<\/p>\n<p class=\"pb-2\">\u201cI\u2019ve hired a full-time HR director to help us because it takes so much time to hire,\u201d said Robin Reich, DDS, a past president of the <a href=\"https:\/\/www.gadental.org\/\" target=\"_blank\" rel=\"noopener\">Georgia Dental Association<\/a> and the founder of <a href=\"https:\/\/www.reichdentalcenter.com\/\" target=\"_blank\" rel=\"noopener\">Reich Dental Centers<\/a>, a dental group with four locations. \u201cWe have to be more flexible. For example, we have two hygienists who are pregnant, and they\u2019re going to job share.\u201d<\/p>\n<p class=\"pb-2\">\u201cAt My Community Dental Centers, we are experiencing this growing demand from our team members for shorter workweeks and alternative schedules,\u201d said Smith. \u201cThe challenge arises from the need to be an adaptable employer and foster a culture that emphasizes work-life balance and well-being while managing a reduced workforce and a surge in patient demand that is at unprecedented levels.\u201d<\/p>\n<p class=\"pb-2\">Multiple DSO leaders cited some of the changes employees and new hires are requesting, including:<\/p>\n<ul class=\"my-2\">\n<li class=\"list-disc ml-8\">Gas stipends<\/li>\n<li class=\"list-disc ml-8\">Flexible work hours<\/li>\n<li class=\"list-disc ml-8\">Work-from-home opportunities<\/li>\n<li class=\"list-disc ml-8\">Higher salaries now instead of generous retirement plans<\/li>\n<\/ul>\n<p class=\"pb-2\">The staffing shortage is causing companies to recruit outside of dentistry. Fischer suggests hiring people for their personalities and skills sets:<\/p>\n<ul class=\"my-2\">\n<li class=\"list-disc ml-8\">Bartenders for front office positions<\/li>\n<li class=\"list-disc ml-8\">Realtors and jewelry store salespeople for treatment presentation and collections<\/li>\n<\/ul>\n<p class=\"pb-2\"><strong>One tip:<\/strong> Evaluate the salaries in your geographic area quarterly to ensure you\u2019re competitive. If the local Starbucks is paying $16 per hour, then you may find it hard to hire somebody for less.<\/p>\n<h3 class=\"pb-2\"><strong>Advice for Dental Practices and Emerging DSOs<\/strong><\/h3>\n<p class=\"pb-2\">\u201cCulture is the first thing. The second is operations: build a great platform. Find those partners that you can grow alongside and really lean in with them,\u201d said Jason Heffelfinger, CEO of Dental Care Alliance.<\/p>\n<p class=\"pb-2\">\u201cFor anyone looking to scale up, take a hard look at what\u2019s made you successful and ensure you have documented those processes. Codify your SOPs so that you can replicate them,\u201d added Williams of Dental Express.<\/p>\n<p class=\"pb-2\">\u201cIf you\u2019re looking to scale up, really sit down and look at your available cash flow,\u201d recommends Fischer, who has worked with more than 1,000 practices. \u201cWhere people get in trouble is when they don\u2019t understand their cash flow. If you do, then you know what you really have to work with and you can expand staff, expand hours, expand services, or expand locations.\u201d<\/p>\n<p class=\"pb-2\">Lastly, take care of your people.<\/p>\n<p class=\"pb-2\">\u201cWe make sure all of our doctor contracts are designed for a minimum of three to five years, and we reward them with a loyalty bonus once they hit those,\u201d explained Hoss Said, managing director of <a href=\"https:\/\/www.mdcmgmt.com\/\" target=\"_blank\" rel=\"noopener\">Massoumi Dental Corporation<\/a>, a dental group with six affiliated practices in Southern California. \u201cOur doctors have to have pride in what they do, and they make their own decisions with clinical autonomy.\u201d<\/p>\n<p class=\"pb-2\">Omid Barkhordar, DDS, and his brother Hamid Barkhordar, DDS, MMSc, are co-presidents of <a href=\"https:\/\/caredentistrygroup.com\/\" target=\"_blank\" rel=\"noopener\">Care Dentistry Group<\/a>, a DSO with 16 affiliated locations in California.<\/p>\n<p class=\"pb-2\">Hamid Barkhordar had this advice for dental groups that are looking to grow: \u201cKeep going. It\u2019s going to be hard.
